How It Works
Consultation. The first step is for you to book a call with us so that we can confirm your eligibility for services. If you're a good fit, we will schedule you for Orientation.
Orientation. During orientation, we will get to know you, complete paperwork, and go over the rules for the program.
Entitlements. After orientation, we will make sure that you are signed up for all of the government benefits you qualify for, such as health insurance, WIC, & SNAP.
Rehabilitation Assessment. Assessments are private and confidential. During this time, we'll sit down with you to discuss your strengths, skills, and needs. In addition, we'll identify your goals and how we can help you achieve them.
Individualized Rehabilitation Plan (IRP). Once we understand what you need, we'll team you up with a specialist who can meet you wherever is most convenient for you: whether it's in your neighborhood, at your home, or here at our office. This specialist will work with you based on your IRP. You'll develop a close relationship with your peer specialist, and they'll be there to support you on your journey to better health and well-being. You can count on meeting with them up to six times each month for coaching and guidance.
