We're looking for a passionate care coordination professional

Must have a High School Diploma or GED and the ability to travel

Qualifications:

  • Have a Maryland State driver’s license
  • Have a reliable vehicle and proof of insurance.
  • Microsoft Office (Word, Excel, PowerPoint, etc.) proficiency
  • Be comfortable meeting clients in their home and community
  •  Strong time management and organizational skills

Responsibilities

  • Collaborate with the Clinical and Executive Directors to create and sustain a healing environment and daily routine for residents.


  • Oversee a diverse group of clients, including children, adolescents, and adults, as a PRP case manager.


  • Organize and lead educational and activity-based groups focusing on nutrition, hobbies, life skills, and coping mechanisms.


  • Execute each client's specific Psychiatric Rehabilitation Treatment plan.


  • Coordinate care by enhancing daily living skills, fostering community engagement, and supporting clients in achieving their goals (e.g., budget management, medication adherence, securing housing).


  • Build and keep up connections with other community service providers.


  • Participate in and contribute to treatment team meetings.


  • Accurately maintain electronic health records, including daily notes, assessments, and monthly reports.


  • Deliver both group and individual services in the facility, community, or at the client’s home, according to their preference.



  • Fulfill any additional tasks as directed by your supervisor.
